Please fill out this quick intake form to get set up with an appointment. We will respond within 24 hours or less to get you scheduled. If this is an urgent request please be sure to indicate that so we can prioritize seeing you. If you have insurance please fill out step 2 as well.
STEP #2 // INSURANCE COVERAGE
We accept many insurance plans. Please use the button below to find out if your plan is one we can bill directly through The Lactation Network. If you are covered that means unlimited lactation care and support for $0!
Not covered? Not to worry, we would love to work with you! Fee for service is due at the completion of the visit. We will provide you with a superbilll (fancy medical receipt) you may choose to submit to your insurance company for reimbursement. See below for more information about pricing.
SERVICES
PRENATAL CONSULTATION
A special appointment to discuss your feeding plans and get all your questions answered before baby arrives. We can discuss topics such as your health history, how to collect and save your milk prenatally if you desire, how to set yourself up for feeding success when baby arrives, breast pump ordering/tutorial, and more. This can be especially helpful if you are a first time mom or have a history of difficulty breastfeeding/pumping in the past.

NEW CLIENT LACTATION/FEEDING
CONSULTATION
During this appointment we will review pertinent health history, discuss your questions/concerns, assess parent/child anatomy including a functional oral assessment of the infant, work through a feeding to evaluate technique and effectiveness, check a pre/post weight feeding if warranted, and create an individualized plan to work towards reaching your feeding goals.
